How to Choose a Primary Care Provider: A Guide for Medicare Patients

How to Choose a Primary Care Provider: A Guide for Medicare Patients

Most advice on choosing a doctor stops at credentials, location, and whether they take your insurance. That’s a fine start, and if that’s the guide you want, our posts on choosing a primary care physician and finding a primary care doctor cover that ground well. But once you’re on Medicare, there’s a bigger question those checklists rarely ask. Here’s what actually matters.

Why This Choice Matters More Once You’re on Medicare

If you’re on a Medicare Advantage plan, your primary care provider is often also a gatekeeper: for referrals, for specialist access, for how your care gets coordinated. Switching mid-year isn’t always simple. If you’re on Original Medicare, you have more flexibility, but the choice still shapes your continuity of care over time.

Plan network rules also play into this decision. For the fuller picture of how Original Medicare and Medicare Advantage differ, see our Medicare Basics guide.

The Question Most Checklists Skip: How Is the Practice Paid?

Here’s what generic “how to choose a doctor” advice, including both of our own posts on that topic, doesn’t cover: how a primary care practice gets paid changes what kind of care you actually receive.

Under fee-for-service, a practice is paid per visit, which can create an incentive to see more patients, more briefly. Under value-based (or risk-based) care, a practice is paid to keep patients healthy, which creates an incentive to invest in prevention, same-day access, and real care coordination.

We go deeper into how this works, including how Palm takes on the medical risk directly, in “What Is Value-Based Care?” Palm’s Approach to Your Health. For now, just know this is a real difference worth asking about, not a minor technicality.

A Practical Checklist for Your First Call or Visit

  • Network & insurance fit: confirm the provider is in-network with your specific Medicare Advantage plan, not just “accepts Medicare” in general. For Original Medicare, confirm they accept assignment.
  • Access: ask about same-day or next-day appointment availability, and how to reach the care team after hours.
  • Coordination: find out whether labs, referrals, and prescriptions are coordinated in-house or if you’ll be managing them yourself.
  • Communication style: is there enough time during a visit to actually be heard, without being rushed?
  • Support beyond the exam room: transportation help, a dedicated support contact, and chronic condition management.

Questions Worth Asking Before You Decide

  • “Is this practice in-network for my specific plan, not just ‘Medicare’ in general?”
  • “How do I reach someone if I have a question after hours or on a weekend?”
  • “If I need a specialist or lab work, how does that get coordinated?”
  • “How much time is typically scheduled for a visit?”

If You Already Have a PCP: When and How to Switch

If you’re on a Medicare Advantage plan, you can typically change your primary care provider within your plan’s network at any time by contacting your plan. That’s different from changing plans entirely, which is tied to specific enrollment periods. For more on those windows, see our Medicare Enrollment Timeline.

What This Looks Like at Palm Primary Care

At Palm Primary Care, we operate on a value-based care model, which is exactly the payment structure we described above. In practice, that means same-day access, an in-house lab, 24/7 support, and a dedicated patient support representative for every patient across the Dallas-Fort Worth area. For the fuller story of how that model works financially, see “What Is Value-Based Care?” Palm’s Approach to Your Health.

Frequently Asked Questions

Does Medicare let me choose my own primary care doctor?

Yes. Under Original Medicare, you have broad flexibility. Under Medicare Advantage, you’ll choose from your plan’s network.

How do I know if a doctor is in-network for my Medicare Advantage plan?

Check your plan’s provider directory, call your plan directly, or ask the practice to confirm. Don’t assume “accepts Medicare” means in-network for your specific plan.

Can I switch primary care doctors at any time?

If you’re on Medicare Advantage, generally yes, within your plan’s network. Switching Medicare Advantage plans is tied to specific enrollment periods.

What areas of the Dallas-Fort Worth area does Palm Primary Care serve?

Palm Primary Care serves patients across the Dallas-Fort Worth area. Visit our locations page for the clinic nearest you.

This article is educational and general in nature. Confirm your specific plan’s network and coverage rules directly with your plan or at Medicare.gov before making a change.
